Job Summary
Care Coordinator at Avanti Homecare (Ilkeston branch). Coordinate weekly rotas for clients and care staff, support onboarding of new care packages and client assessments, maintain accurate records across digital systems, liaise with clients, families and external professionals, and participate in an on-call rota (paid in addition to salary). On-call schedule is one week in four, with evenings until 10:30pm; office hours 8:30am–4:00pm, Monday–Friday. Requires strong organisation, clear communication, empathy, reliability, IT proficiency, and a UK driving licence. Salary £28,700, full-time permanent contract.
Required Qualifications
- A minimum of one year’s experience supporting and caring for people
- At least 6 month’s experience in an office-based role involving rota planning with digital systems
- Strong IT skills, ideally including digital care planning systems
- Strong experience in working with Microsoft Office or Google based software (or similar), email writing and managing an online diary
- A Level 3 Diploma in Health and Social Care or working towards completion
- A full UK driving licence and access to your own vehicle, with the ability to travel to clients’ homes when needed
- Living within a reasonable travelling distance of the Ilkeston branch
