Job Summary
Determine and document patient care needs, analyze HIMS records to identify diagnostic and health maintenance gaps, and discuss these with patients to order and coordinate follow-up appointments and studies. Review discharge summaries for emergency or hospital patients, update records, schedule provider visits, and contact patients to ensure discharge instructions are followed. Assist chronic disease patients by coordinating education, durable medical equipment, and home health services while managing complex care plans and urgent referrals through social service platforms. Collaborate with clinic staff to ensure quality benchmarks are met, populate insurance metrics, and facilitate Medicare wellness visits.
Required Qualifications
- At least one (1) year of experience in a similar position with knowledge of principles that relate to the position, including strategies to resolve differences between care that patients receive and care that patients need ("care gap management"), considerations that are significant as patients change from one care level or setting to another ("patient transition"), and familiarity with community resources and healthcare networks that are available for patient referral.
- Possession of experience in use of electronic health information management systems (HIMSs), skill in multitasking and time management, and ability to manage complex care coordination duties.
- Aptitude in verbal and written communication, including superior ability to effectively interface with patients and healthcare teams.
- Completion of a postsecondary medical assisting training program at an accredited school.
- Possession of current certified or registered medical assistant certification by the American Association of Medical Assistants, American Medical Technologists, National Center for Competency Testing, or National Healthcare Association.
- Possession of United States proof of citizenship or right to work in the United States.
- Possession of a driver license or analogous identification.
- Willingness to participate in Hospital orientation and educational in-service.
- Monday – Friday, 8:00 PM – 4:30 PM
- based in our Aumsville Medical Clinic location
Desired Qualifications
- Optimally, possession of an associate degree from an accredited institution in a field related to healthcare administration, or possession of certification in such a field.
- Optimally, completion of other education related to leadership.
- Optimally, possession of other pertinent and current certification related to leadership.
