Job Summary
Provide non-clinical administrative support to the Care Coordination process, including timely discharge planning and utilization management. Communicate with internal and external customers while maintaining data integrity and adhering to federal, state, and managed care guidelines. Transition patients through discharge planning to enhance satisfaction and work effectively with nursing and medical staff to relay pertinent information efficiently. Maintain professional, courteous service regarding confidential issues and contribute to the overall team effort.
Required Qualifications
- H.S. Diploma or GED
- 2 years' experience in hospital care coordination/case management, home health, quality improvement or a related field
Desired Qualifications
- Some college coursework
