Job Summary
Provide non-clinical administrative support for the Care Coordination process, including timely discharge planning and utilization management. Maintain data integrity while ensuring compliance with federal, state, and managed care guidelines. Facilitate transition and care coordination processes to enhance patient and family satisfaction. Communicate effectively with internal and external customers, nursing, and medical staff to relay pertinent information promptly. Uphold professional standards by handling confidential issues with integrity and tact. Work effectively with peers to maintain a positive team environment. Requires an H.S. Diploma, two years of experience in care coordination or case management, and some college coursework preferred.
Required Qualifications
- H.S. Diploma or GED
- 2 years' experience in hospital care coordination/case management, home health, quality improvement or a related field
Desired Qualifications
- Some college coursework
