Job Summary
Care Coordinator and Case Manager for adolescents with mental health and behavioral health needs. Lead and coordinate Child and Family Team meetings, develop ISPs, provide case management, and engage families using trauma-informed, person-centered approaches. Build relationships with clients, families, and multidisciplinary teams; connect with natural supports and community resources; promote Wraparound processes; ensure HIPAA compliance, accurate documentation, and safety in a structured treatment environment. Eligible candidates will have a bachelor’s degree in a related field (master’s preferred), at least 2 years of relevant mental health/case-management experience, strong care coordination and treatment-planning skills, experience with youth and trauma-informed models, ability to manage multiple cases, and a valid NC driver’s license with acceptable driving record. The role involves standing, walking, and responding to behavioral crises, including potential safety interventions, in a supportive behavioral health setting.
Required Qualifications
- Bachelor�s degree in Human Services, Social Work, Psychology, or related field (Master�s preferred)
- Minimum 2 years of experience in mental health, case management, or youth services
- Strong skills in care coordination, treatment planning, and team collaboration
- Experience working with adolescents, families, and trauma-informed models preferred
- Ability to manage multiple cases in a fast-paced environment
- Must be 21 years or older
- Valid North Carolina driver�s license with good driving record
- Demonstrates effective emotional regulation and professionalism
- Must pass background check and drug screening
