Job Summary
Bilingual Spanish/English Intensive Care Coordinator (ICC) in WestCoast Children’s Clinic Oakland leads outreach and engagement for foster youth and families, providing community-based trauma-informed services, intensive skill-building for independent living, assessments, crisis intervention, stabilization and safety planning, and connection to housing, medical, mental health, education, employment, advocacy, and placement stability. Works collaboratively with a multidisciplinary team to engage youth and families, perform initial contact and intake, and deliver targeted case management and community-based rehabilitation services in a mix of virtual and in-person environments. Requires lived experience with relevant systems, strong documentation, and the ability to travel within about 90 miles of the office. Bilingual Spanish/English required; ALTA language proficiency assessment will be administered for final candidates.
Required Qualifications
- Direct, lived experience with foster care, social services, juvenile justice, mental health, residential treatment or group home placements, wraparound services, or other related community resources
- Bilingual in English and Spanish (required)
- Bachelor's degree in Psychology, Criminal Justice, Social Work or related field (preferred)
- Experience providing case management and rehabilitation services
- At least 2 years experience working with system-involved youth
- Demonstrated ability to work with families who have experienced high levels of trauma and demonstrate high mental health needs
- Ability to work independently and collaboratively as part of a multidisciplinary team
- Proven ability to be organized, manage time well, complete high quality and timely documentation
- Ability to travel by car to meet with clients, within ~90 mile radius; CA DMV record and vehicle required
