Care Coordinator - African American Healing Center (AAHC)

Facilitate individual and group counseling sessions, including process groups, psychoeducational groups, and relapse prevention curricula. Perform crisis intervention, accompany clients to offsite appointments, and proactively link clients to internal and external resources while following up on progress. Collaborate with caseload clients to develop and maintain treatment plans, progress notes, and discharge paperwork, ensuring accurate documentation in electronic systems. Complete required trainings, attend team meetings, and plan client celebrations. May work weekends and holidays as needed. This role supports the African American Healing Center's mission to prevent violence and honor cultural context through integrated physical and behavioral health services.
