Job Summary
Establish and maintain professional relationships with individuals, families, and community partners while completing initial and ongoing assessments to identify risk factors. Coordinate services across healthcare, behavioral health, housing, and social systems by facilitating communication among providers and scheduling appointments to reduce care barriers. Provide targeted case management and supportive interventions consistent with scope of practice, advocating for access to appropriate resources and ensuring timely, compliant documentation in the electronic health record. Requires a valid driver's license, proficiency with EHR and Microsoft Office, and ability to travel frequently within the service area for community-based work.
Required Qualifications
- Possesses the equivalent of formal training in behavioral health, usually in the form of a major in Psychology, Social Work, Counseling, or Criminal Justice as part of a Bachelor's Degree program.
- Maintains current knowledge of standards of care and practices, typically acquired through continuing education.
- A valid state-issued driver's license is required.
- Displays basic organizational skills to self-prioritize and organize own work.
- Determines effective responses to situations encountered within established precedent.
- Able to effectively communicate opinions drawn from conclusions using inference and logic.
- Exercises tact and diplomacy in resolving mild conflicts or disagreements.
- Requires frequent travel within the service area.
- Physical activities may include walking, standing, climbing stairs, and occasional lifting of materials.
