Job Summary
Care Coordinator role at WellLife Network focusing on collaborative client care across health and social services. Responsibilities include outreach to individuals with chronic conditions or co-occurring disorders, conducting initial and ongoing assessments, developing and revising care plans, maintaining monthly contact via phone and in person, advocating for resources, coordinating insurance/linkages to treatment, collaborating with multidisciplinary teams, crisis intervention when needed, detailed EMR documentation, and participation in team meetings and trainings.
