Care Coordinator - 0006N - Mon-Fri 9AM-5PM

Conduct outreach activities to engage individuals with chronic medical conditions, mental health issues, and substance use disorders. Perform comprehensive assessments to determine client strengths and needs, then prepare and revise care plans aligned with personal goals. Maintain monthly contact via telephonic and face-to-face outreach to coordinate care, manage medical insurance access, and link clients to community resources. Advocate for clients to decrease emergency room visits and inpatient admissions while conducting crisis interventions as needed. Monitor and update electronic medical records, complete monthly documentation for billing, and collaborate with community providers in weekly team meetings. Report daily office and field schedules to the supervisor and attend periodic supervision and trainings. Maintain agency vehicles, cell phones, and laptops while adhering to program guidelines and confidentiality standards.
