Job Summary
Conduct outreach activities to engage individuals with chronic medical conditions, mental health issues, and substance use disorders while maintaining monthly contact through telephonic and face-to-face interactions. Perform comprehensive assessments to determine client needs, prepare care plans, and monitor coordination of medical insurance and community resources to decrease emergency visits. Collaborate with multi-disciplinary teams for goal-directed care planning, conduct crisis interventions when needed, and maintain detailed electronic medical records. Complete monthly documentation for billing, attend weekly team meetings, and participate in bi-weekly supervision. Manage agency vehicles and equipment, report issues to administration, and follow program guidelines.
Required Qualifications
- Bachelor's degree
- two years' experience in Human Services
- New York State driver's license
- access to a vehicle
