Job Summary
Coordinate care for clients with chronic medical conditions and co-occurring issues; conduct initial and ongoing assessments; prepare and revise care plans to reflect client needs and goals; provide monthly and as-needed outreach via phone and in-person visits; advocate for access to resources and coordinate with providers to support wellness and reduce ER visits; maintain detailed EMR documentation; collaborate with a multidisciplinary team to ensure goal-directed care planning; conduct crisis intervention when needed; manage scheduling and field activity in coordination with supervisors; ensure insurance linkage and community resource access; travel as required; must have a Bachelor’s degree, two years in Human Services, and a NY driver’s license with access to a vehicle.
Required Qualifications
- Bachelor’s degree and two years’ experience in Human Services
- New York State driver’s license and access to a vehicle
