Job Summary
The Care Coordination Program Manager leads and supervises the Care Coordination team across Steamboat Springs and Craig, partnering with regional healthcare providers and RMHP to deliver high-quality, accessible care coordination services. Oversees program delivery, budgeting, grants coordination, and data-driven performance metrics; develops protocols, fosters cross-organizational collaboration, and advances health equity in rural Northwest Colorado. Requires strong leadership, project management, stakeholder engagement, effective communication, program development, and the ability to manage multiple priorities while aligning with THP’s strategic plan.
Required Qualifications
- Bachelor’s degree or relevant experience required
- Minimum of one or two years of clinical and/or care coordination experience
- Experience supervising or managing employees
- Strong background in customer service, case management, care coordination, or related human services work preferred
- Analytical skills for data tracking, reporting, documentation accuracy, program metric review, and process improvement
- HIPAA-compliant practices preferred
- Ability to travel independently throughout the region, including during inclement weather conditions
- Valid driver’s license and automobile insurance coverage of at least $300,000 per occurrence
