Job Summary

The Care Coordination Program Manager will provide programmatic supervision of the care coordination team and ensure adherence to the high-fidelity wraparound model of care. Responsibilities include delivering trauma-informed, child- and family-centered services, developing and implementing training plans for care coordinators, advocating for children and families within systems of care, collaborating with schools and community partners, maintaining fidelity monitoring and budget oversight, ensuring compliance with regulatory standards, and supervising eight direct reports with regular supervision. The role requires establishing and maintaining professional relationships across diverse communities, supporting continuous quality improvement, and coordinating with program leadership to achieve positive outcomes for children, youth, and families. Qualifications include a Master’s Degree in Social Work, Counseling, Clinical Psychology, or related area and current Ohio licensure (LSW, LPC, or MFT), with independent licensure preferred; minimum two years of experience with children and families; strong communication, documentation, and technical skills (Microsoft Office, EHR familiarity), and the ability to manage multiple priorities in a dynamic environment.

Required Qualifications

  • Master’s Degree in Social Work, Counseling, Clinical Psychology, or related area of Human Services is required
  • Current Licensure in the State of Ohio (LSW, LPC or MFT) is required
  • Independent Licensure in the State of Ohio (LISW, LPCC or IMFT) is preferred
  • Minimum of two years’ experience working with children and families is required
  • Experience working in community-based services is preferred
  • Valid Ohio driver’s license and current auto insurance is required
  • Excellent verbal and written communication skills
  • Proficiency with Microsoft Office products (Word, Excel, Access, Outlook) and familiarity with Google Maps/GPS
  • Ability to learn and utilize different technology systems including an Electronic Health Record
