Care Coordination Master Social Worker

Job Summary
Conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. Advocate for clients and collaborate with multidisciplinary teams to ensure holistic support, including safe discharge planning and continuity of care for diverse populations. Manage psychosocial issues such as abuse, grief, and substance abuse while coordinating transitions to post-acute care and community resources. Facilitate patient/family care conferences and educate stakeholders on health management, bioethics, and insurance programs like CCS/Medicare/Medicaid/SSI. Support staff and patients in addressing social determinants of health, including housing and food insecurity, within the Virginia Mason Franciscan Health network across the Puget Sound region.
Required Qualifications
- Masters Other Social Work and 1-Year Post-MSW experience or Social Work internship in a clinical or medical setting
- Licensed Independent Clinical Social Worker: WA
- Licensed Advanced Social Worker: WA
- Licensed Social Worker Associate Advanced:WA
- Master Social Worker: WA
