Job Summary
Care Coordination Manager oversees the development and daily operations of care coordination services. This role leads care management initiatives focused on improving patient outcomes, advancing population health, and reducing healthcare costs through data analytics, policy development, quality improvement, and cross-department collaboration. The Supervisor supports high-risk patients by ensuring coordinated, comprehensive care across healthcare settings and promoting best practices to improve continuity of care and close healthcare gaps.
Required Qualifications
- High School Diploma or GED Equivalent required
- Associate degree or higher in a relevant subject area such as public health, nursing, health administration, social work, business administration preferred; equivalent combination of education and relevant work experience may be considered
- 2 to 5 years’ experience in population health and underserved populations
- Electronic Medical Record competency and proficiency
- Effective interpersonal skills and an understanding of the interdependent roles of various allied health professions
- Bilingual fluency in Spanish strongly preferred
- Commitment to social justice and health equity
