Job Summary
Care Coordination Manager responsible for developing and overseeing Life Plans for individuals with intellectual and developmental disabilities, coordinating medical, behavioral health, and community services, ensuring Medicaid eligibility, and utilizing person-centered planning processes. Role involves a hybrid work model with in-office, field, and remote work, and requires engagement with families, advocates, and interdisciplinary teams. Key duties include comprehensive assessments, life plan development and review, team coordination, documentation, HIPAA and IT security adherence, and ongoing participant support to maximize independence and satisfaction. Qualifications include Bachelor’s degree with relevant experience (or Master’s with relevant experience), RN license with relevant experience, valid driver’s license, strong communication and computer skills, and commitment to equitable, person-centered care across diverse populations.
