Job Summary
Conduct comprehensive assessments and develop individualized Life Plans for people with intellectual and developmental disabilities. Coordinate interdisciplinary team meetings, ensure Medicaid and benefit eligibility, and monitor health and safety compliance. Utilize CQL interview techniques to inform care plans and maintain accurate documentation across electronic systems. Work with families and advocates to support informed choice and consumer satisfaction while adhering to HIPAA and IT security policies. This hybrid role requires a Bachelor's or Master's degree, RN license, and valid driver's license. Southern Tier Connect offers a competitive hourly rate between $25 and $26, with a 401k match up to 5% and generous paid time off.
