Job Summary
Manage timely post-acute care referrals to assist with length of stay management and mitigation of denials. Collaborate with the Care Coordination team, including Registered Nurse Care Coordinators and Social Workers, to facilitate care progression across all patient age groups. Ensure post-acute follow-up appointments for identified patients to assist with readmission prevention and continuity of care. Deliver required patient notifications and letters, and obtain patient or family signatures on documentation to meet regulatory compliance. Serve as the primary receptionist and secretary while supporting safe, therapeutic patient care through multidisciplinary processes. This role supports the Care Coordination team at Chandler Regional Medical Center, a 443-bed acute care hospital within Dignity Health.
Required Qualifications
- Associate (two-year) Degree in Business/Health-Related field or High School Diploma or GED
- Medical terminology certificate or ability to demonstrate knowledge of medical terminology
- Intermediate to advanced computer skills required to include Google Workspace applications including Docs, Sheets, typing, and data entry skill
Desired Qualifications
- At least 3 years previous experience in health care, care coordination, utilization review, discharge planning or similar healthcare setting
- Bachelor's degree or BSW
- Experience supporting clinicians in demanding and fast paced patient care environments
