Job Summary
Manage post acute care referrals, communicate directly with patients and families, and coordinate with post acute care agencies and key stakeholders. Perform these duties with high accuracy using critical thinking skills while adhering to hospital policies, standards of practice, and Federal and State regulations. Maintain a high degree of precision in all interactions to support the Clinical Care Coordination team's time-sensitive functions.
Required Qualifications
- High School Diploma/GED
- current license/certification as a healthcare professional (Medical Assistant/Certified Nursing Assistant/Emergency Medical Technician)
- Associate (two-year) Degree in Business/Health-Related field
- Medical terminology certificate
- ability to demonstrate knowledge of medical terminology
- Must be able to lift 50lbs force occasionally
- Must be able to lift 20lbs force frequently
- Must be able to lift 10lbs force constantly
Desired Qualifications
- At least 3 years previous experience in health care, care coordination, utilization review, discharge planning or similar healthcare setting
- Bachelor's degree or BSW
- Experience supporting clinicians in demanding and fast paced patient care environments
