Job Summary
Care Coordinator for pediatrics coordinates healthcare services for patients and families, facilitates transitions of care, and promotes adherence to treatment plans. Works with providers, payers, and community resources to deliver patient-centered care. Responsibilities include monitoring patient progress, facilitating communication between patients and clinical teams, maintaining accurate records in care management systems and EHR, ensuring regulatory compliance, assisting parents in understanding care plans and resources, coordinating appointments and referrals, connecting patients with community services and financial assistance, and educating on preventive care. Requires strong communication, multitasking, and proficiency with care management software/EHR. Supervises Care Coordination staff. Travel may be required, with frequent driving. Education/experience: Bachelor’s degree in nursing, LPN certification, or related field preferred; 1-2 years in care coordination or case management and/or patient services preferred. Working conditions are general office with occasional overtime; physical demands include mobility and travel. Location: Jacksonville, FL.
