Job Summary
Care Continuum Partner serves as the main liaison between the patient, family, and clinical team across the continuum of care. In collaboration with the clinical care team, the role participates in pre-visit planning to ensure smooth patient flow and coordinates scheduling for outpatient procedures, imaging, labs, and other services. The partner handles referral management and intake, precertification processes, and follow-up communications via telephone, patient portal, and other virtual platforms. Responsibilities include coordinating and scheduling complex appointments, creating and maintaining a robust care continuum plan, ensuring care compliance, providing new patient coordination, outreach to follow up on patient needs, and connecting patients and families with available internal resources to support the continuum of care. The position emphasizes exceptional customer service, clear patient instructions, and effective communication among physicians, care teams, and support services, while maintaining confidentiality and adapting to changing priorities.
Required Qualifications
- High School Diploma/GED
- 2 years experience interacting with patients
- 3 years experience in a complex health system environment or related field
