Job Summary
Conduct monthly in-person rounds for patients enrolled in Advanced Primary Care Management or the Guiding an Improved Dementia Experience programs. Address patient and durable power of attorney inquiries via telephone and respond to messages through the Curana Patient Portal. Manage electronic health records, coordinate documents for provider review, and facilitate orders while assisting with prior authorizations. Collaborate with the Interdisciplinary Care Team to ensure seamless communication within the health network. Work on-site at Senior Living Facilities four days per week, with potential travel to multiple local communities.
Required Qualifications
- Registered or Certified Medical Assistant
- 1+ years of experience working in a medical office, community engagement, or other related fields
- Ability to interpret medical records, lab results, and appointment notes
- Demonstrates experience in collecting clinical information and surfacing changes to the appropriate Curana care team member
- Willingness to travel to local Senior Living Communities
- Proficient computer skills and exhibits knowledge in using various Microsoft Office applications
- Excellent organizational and time management skills with the ability to prioritize tasks
- On-site at assigned Senior Living Communities 4 business days out of the work week
- May require travel to multiple local Senior Living Communities
- This role requires travel to and from one or more assigned facilities
- Depending on the amount of travel required, Curana may in future determine that a valid driver's license, current auto insurance, and an acceptable driving record are required
Desired Qualifications
- Experience working with chronic care management (CCM) or other care management programs
