Job Summary
Conduct home visits to caregivers in the assigned Indiana territory to provide support, locate resources, and build goodwill. Coordinate mutually convenient visit times with clients, document notes in required apps, and maintain compliance records per state guidelines. Communicate regularly with the Care Coach Supervisor and attend monthly virtual team meetings. Travel weekly throughout the cluster, utilizing your own computer and tablet for documentation. Maintain confidentiality under HIPAA regulations and adhere to company policies while treating clients with compassion and integrity.
Required Qualifications
- High school diploma or equivalent
- Strong understanding of the emotional and physical demands placed on caregivers
- Experience working with medically complex populations
- Must be detail oriented
- Demonstrate good organizational skills and time management skills
- Must have the ability to travel on a weekly basis as this role is a field-based position
- Must have an active driver license and reliable transportation with insurance to drive throughout the day / territory
- Ability to effectively communicate with patients and families
- Strong critical thinking and problem-solving skills
- US work authorization (Required)
- Must have your own computer/ iPad or tablet and cell phone
