Job Summary
Provide direct personal care for elderly, convalescent, or disabled persons in home or facility settings, including assistance with activities of daily living, housekeeping, laundry, meal preparation, and shopping. Deliver care as directed by nurses according to established plans, while maintaining accurate documentation and reporting status changes to supervisors. Utilize equipment effectively, adhere to infection policies, and coordinate with multidisciplinary teams to ensure efficient workflows. Maintain confidentiality and uphold safety standards while supporting patients through social activities prescribed by their care plans.
Required Qualifications
- Drivers License - Licensing Board
- Certified Nurse Aide (CNA) - Licensing Board
- High School or GED Equivalent
- Active license in the state of practice
- Less than 1 year in home health care or related health care field
- Visual acuity adequate to perform position duties
- Ability to communicate effectively with others
- Ability to hear, understand and distinguish speech and other sounds
- Ability to work the hours specified
- Minimum of 4 shifts per month required
