Job Summary
Care Advocate provides non-clinical patient navigation support for Medicare patients, following an intake with a clinical lead, by helping patients understand their care plans, schedule appointments, navigate insurance, and connect to community resources. The role involves removing friction in care journeys, coordinating with clinical team members, documenting interactions, and maintaining HIPAA/privacy compliance while managing multiple patients as a remote 1099 independent contractor, typically 20–40 hours per week.
Required Qualifications
- Active, unrestricted LPN/LVN license in good standing
- Community Health Worker (CHW) certification
- or equivalent community health / patient-navigation experience
- 2+ years of patient- or community-facing support experience (care coordination, case management, member advocacy, health coaching, benefits navigation, community health work, or similar)
- Working knowledge of the U.S. healthcare system and Medicare
- Comfort working independently, managing a caseload, and maintaining strong documentation
