Job Summary
Care Advocate for Care Delivery (Wider Circle) executes person-centered care plans under PIN and CHI programs, bridges clinical care with social needs, conducts SDOH assessments, coordinates care and post-discharge support, navigates Medicare CMS documentation, and connects patients with local NJ resources. Responsibilities include conducting assessments, coordinating with clinicians and billing practitioners, managing appointments and transportation, updating patient charts in CMS, and providing motivational interviewing and behavioral support to patients and families. Requires 3+ years in community outreach or healthcare, CHW training preferred, strong digital literacy, ability to work remotely within a highly independent remote team, and willingness to drive within a 50-mile radius in New Jersey. Must have a high school diploma/GED (college degree preferred), valid driver’s license, reliable transportation, and flexible schedule for occasional after-hours work. Benefits include comprehensive health coverage, 401(k), PTO, FSA/HSA options, and compensation of $23.36-$26.82 per hour.
Required Qualifications
- CHW training or certification preferred
- Equivalent experience in community health, social services or related field
- CMS-mandated training and state-applicable requirements
- 3+ years of relevant community outreach, facilitation, or healthcare experience
- Strong computer skills and ability to navigate web-based and app-based systems
- High school diploma/GED (college degree preferred)
- Driver’s license, reliable transportation, flexible schedule
