Job Summary
Answer incoming calls promptly according to SOP and training guidelines, accurately identifying patient needs and communicating effectively with patients, caregivers, and healthcare providers. Triage and transfer cases appropriately to RNs, providers, and emergency services while charting in the EMR according to national standards. Motivate patients toward proactive health management, provide education on self-management goals, and facilitate referrals to internal teams, payer programs, and community resources. Assist with scheduling PCP, virtual clinic, or specialist appointments as necessary. Support after-hours and 365-day call queue operations with slotted shifts including weekends and weeknights.
Required Qualifications

- Weekends, 9a-9pm, 2 weekends/ month minimum on average
- Weeknights, 5pm-9pm, approximately 20 hours per week
- Optional and flexible in 2, 4, 6 and 8 hour increments
- Every 3rd Holiday 5pm -9pm at a minimum
- minimum of 5 MBPS for Download Speed
- 3 MBPS for the Upload Speed
