Job Summary
Perform comprehensive clinical assessments, including volume status evaluations and outpatient IV diuretic management, to optimize guideline-directed medical therapy for complex heart failure patients. Coordinate seamless post-discharge follow-up care and interdisciplinary treatment plans to strengthen continuity and prevent hospital readmissions. Serve as a clinical leader and mentor for Heart Failure Center RNs, collaborating closely with community cardiologists, pharmacists, and care coordinators to execute individualized patient care plans. This role is part of an established team of 10 physicians and 7 APPs within Ascension Saint Agnes Heart Failure Clinic, a nonprofit facility serving a population of approximately 738,000. The position offers full-time outpatient employment with no call structure and includes a base salary ranging from $122,587 to $153,376 per year, along with CME allowance and malpractice coverage.
Required Qualifications
- Nurse Practitioner with Prescriptive Authority credentialed from the Maryland Board of Nursing obtained prior to hire date or job transfer date required
- Registered Nurse credentialed from the Maryland Board of Nursing obtained prior to hire date or job transfer date required
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required
- American Heart Association or American Red Cross accepted
- Master's degree of Nursing required
- Board Certified in Acute Care
- Eligible for state medical licensure
- DEA registration
