Cardiology APP - Heart Failure Clinic (NP/PA)

Evaluate and manage patients referred to the Heart Failure Bridge Clinic, developing individualized treatment plans and collaborating with physicians on complex cases. Assess volume status to determine appropriate interventions, including IV diuretic therapy when indicated, while coordinating transitions of care between inpatient and outpatient settings. Provide clinical support and guidance to HFBC nursing and support staff, serving as a liaison between heart failure specialty clinics, inpatient teams, and multidisciplinary providers. Ensure care delivery aligns with established clinical protocols and quality standards, and document and bill for professional services provided. This full-time, 4-day, 10-hour weekday role offers outpatient-focused care with an emphasis on patient education and care coordination within Johns Hopkins' Advanced Heart Failure Program.
