Cardiology APP - Heart Failure Clinic (NP/PA)

Evaluate and manage patients referred to the Heart Failure Bridge Clinic, developing individualized treatment plans and collaborating with physicians on complex cases. Assess volume status to determine appropriate interventions, including IV diuretic therapy, while coordinating transitions of care between inpatient and outpatient settings. Provide clinical support to nursing staff and serve as a liaison between specialty clinics and multidisciplinary providers, ensuring care aligns with established protocols. Document and bill for professional services provided. This full-time, 4-day, 10-hour weekday role excludes weekends and holidays. Candidates must hold current Maryland NP or PA licensure with prior cardiology experience.
