Cardiac/Stroke Clinical Nurse Part Time Nights

Provides direct patient care activities including assessment, diagnosis, planning, implementation and evaluation within standards of nursing care. Delivers patient- and family-centered care, educates patients and families on illness management and post-treatment home care, and ensures patient safety, quality, and regulatory compliance. Develops, implements and documents individual care plans with goals in collaboration with the interprofessional team, patient, family or caregiver; communicates plans of care across the continuum. Applies critical nursing skills to assess physical, psychosocial, emotional needs and respects patient values and cultural considerations.
