Cardiac/Stroke Clinical Nurse

Provides direct patient care activities including assessment, diagnosis, planning, implementation and evaluation within the standards of nursing care. Promotes patient and family centered care in a healing environment and educates patients and families on post-treatment home care needs and medication administration. Develops, implements, and documents individual plans of care in collaboration with the interprofessional team, patient, family or caregiver. Uses critical nursing skills to assess and address physical, psychosocial, emotional, cultural, and population-specific needs. Complies with safety-sensitive requirements as an essential function of the job.
