Job Summary
Hybrid Digital Health Specialist and Physical Therapist role delivering telehealth, remote therapeutic monitoring (RTM), and in-clinic care. Develop and deliver personalized plans of care for a diverse orthopedic and neurologic patient population; objectively measure outcomes using cutting-edge software; efficiently document evaluations, treatments, re-evaluations, and discharge notes. Utilize a digital platform to deliver telehealth visits and RTM services; collaborate with treating clinicians, liaise with the Director of Digital Health, and support program development and marketing to grow remote services. Strong emphasis on delivering high-quality care through digital health tools, EMR automation, and data analytics; contribute to regional service metrics and ongoing professional development.
Required Qualifications
- Diploma from a CAPTE-approved Physical Therapy Program
- Physical Therapy state license (or in process)
- Digital/telehealth experience preferred!
