CAP Registered Nurse (RN) – Part-Time/PRN

Conduct in-home nursing assessments for beneficiaries in the Community Alternatives Program, including initial, annual, Continued Need Review, and Change of Status evaluations. Evaluate medical, psychological, social, and environmental needs to develop person-centered care plans and recommend interventions. Update documentation in the electronic medical record, gather provider records, and review critical incidents while collaborating with Case Managers and families. Ensure all nursing documentation meets state, CAP, and audit requirements. Requires a current North Carolina RN license, reliable transportation, and ability to work independently with frequent driving and travel. Unit-based compensation includes $43.75 per assessment unit plus travel and mileage reimbursement.
