Job Summary
CAP Nurse Case Manager (RN) provides assessments, care planning, and care coordination for individuals with intellectual and developmental disabilities and mental health/substance use needs in Onslow & Duplin areas. Responsibilities include conducting comprehensive assessments across medical, behavioral, social, and environmental domains; educating families about waiver services and Medicaid; developing and updating person-centered care plans; coordinating services (personal care, waiver supplies, home modifications, educational support); monitoring services via monthly contacts and home visits; offering counseling and support to beneficiaries and families; ensuring Medicaid and program compliance; and participating in state-required training. Requires RN license in NC, nursing degree (associate or higher), and NC Medicaid-certified training within 90 days (or apprenticeship pathway). Must be able to lift 10 lbs and perform physical tasks as needed. Flexible hours with part-time status.
Required Qualifications
- Nursing background with current North Carolina RN license
- Two-year or four-year nursing degree (associates or BSN)
- NC Medicaid-certified training within 90 days or apprentice pathway within supervision
