Job Summary
CAP Nurse Case Manager (RN) coordinates care for individuals with intellectual and developmental disabilities, mental health, and substance use challenges. Performs assessments across medical, psychological, behavioral, financial, social, cultural, environmental, legal, vocational, and educational areas; educates beneficiaries and primary caregivers about waiver services, Medicaid, and community resources; develops and maintains person-centered care plans updated at least annually or when status changes. Coordinates services (personal care, waiver supplies, home modifications, educational support) to align with the care plan, monitors service quality, ensures Medicaid/program compliance, and facilitates access to additional services to prevent institutionalization. Provides counseling and support to families, documents beneficiary progress through regular follow-ups, and ensures accurate records in accordance with Medicaid and program requirements. Maintains ongoing collaboration with an interdisciplinary team, advocates for participants, and participates in required CAP program trainings and compliance activities. Requires ability to lift at least 10 lbs, move up to 20 lbs, and perform physical tasks as part of the role.
Required Qualifications
- Current North Carolina Registered Nurse (RN) license
- two-year or four-year nursing degree
- one year of case management experience in home care, long-term care, personal care, or related work
- completion of an NC Medicaid-certified training program within 90 calendar days of employment
