Job Summary
CAP Case Manager provides in-home, person-centered case management for beneficiaries with intellectual/developmental disabilities, mental health, and substance-use needs. Responsibilities include conducting comprehensive assessments across medical, psychological, behavioral, financial, social, cultural, environmental, legal, vocational, and educational domains; educating families about waiver services and Medicaid resources; creating and updating individualized care plans; coordinating services and referrals; monitoring progress through regular home visits and follow-up communications; maintaining accurate documentation to ensure regulatory compliance; collaborating with RNs and an interdisciplinary team to advocate for participants and secure needed resources. Travel is required, with work conducted within an hour’s travel of Onslow and Craven Counties, NC.
Required Qualifications
- Bachelor’s degree in social work from an accredited school of social work OR Bachelor’s degree in a human services or equivalent field OR RN with case management experience; NC Medicaid-certified training within 90 days of employment
