Job Summary
Coordinate Coordinated Access Network functions with participating agencies, troubleshoot system problems, and supervise front-line staff for assessments, housing referrals, and data collection. Monitor shelter stand-by lists to ensure bed availability, facilitate Housing Placement Team Meetings, and manage the MMW By-Name List by cross-checking registries with the HMIS database. Report updates to the CAN Steering Committee, maintain communication with 2-1-1 administrative staff, and oversee data quality for the CT Balance of State and regional committees. Supervise homeless outreach programs and warming centers when applicable. Full-time, exempt role with a Bachelor's degree in social services or related field required.
