Job Summary
ECM Nurse Case Manager coordinating care for CalAIM ECM/CS members; conducts needs assessments, builds personalized care plans with TIC and Harm Reduction principles, coordinates with doctors, therapists, and social workers, assists with medication reconciliation, pulls outside medical records, attends specialist appointments with members, ensures documentation and billing readiness, coordinates discharge planning for hospitalized or in-patient members, and supports home/community-based services; role includes in-person visits, outreach, and collaboration to ensure member safety and stability.
Required Qualifications
- LVN from an accredited school
- Current California Driver’s License and proof of auto insurance
- Eligible to work in the US
- Must be 18 years of age or older
- Minimum two years of experience working with individuals with complex needs
- Minimum two years experience working in the nursing field
- Experience in case management
- Reliable transportation and ability to transport members occasionally
- Must pass a relevant background check as a Medi-Cal provider upon provisional offer of employment and maintain eligibility
