Job Summary
CalAIM Case Manager coordinates care for clients enrolled in CalAIM ECM and CS programs, conducting in-depth needs assessments, developing personalized care plans, coordinating with providers, performing home visits, engaging in community outreach, and documenting interactions to ensure high-quality, trauma-informed, client-centered care. The role blends office work with community outreach, requires collaboration with a care-management team, and emphasizes culturally competent service delivery, eligibility for billing, and ongoing professional development.
Required Qualifications
- Bachelor's degree in social work, public health, or a related field or equivalent experience
- One year of experience case managing individuals with complex needs
- Current California Driver’s License and proof of auto insurance
- Eligible to work in the US
- Must pass a relevant criminal background check as a Medi-Cal provider and a driver background check upon provisional offer of employment and maintain eligibility
