Job Summary
Lead a team of care managers delivering CalAIM ECM and CS program care coordination; supervise and mentor staff; develop and implement processes for client/staff outcomes, financial sustainability, and high integrity; conduct needs assessments and develop personalized care plans using Trauma-Informed Care and Harm Reduction; coordinate care with doctors, therapists, social workers, and other providers; conduct home visits and community outreach; advocate for client needs; manage documentation and billing readiness; oversee outreach van and shower trailer operations; requires 32-hour week, hourly rate $43.75, and a benefits package including health coverage and retirement plan.
Required Qualifications
- Bachelor's degree in social work, public health, or a related field or equivalent experience.
- Minimum two years of experience providing case management to individuals with complex needs.
- Must pass a relevant background check as a Medi-Cal provider upon provisional offer of employment and maintain eligibility.
- Current California Driver’s License and proof of insurance.
- Reliable transportation and ability to transport members occasionally.
- Must be 18 years of age or older.
