Job Summary
Independent licensed mental health therapists (LCSW, LMFT, CMHC) are sought to provide in-person diagnostic evaluations, psychotherapy, treatment planning, and consulting in SNF/LTC facilities in Utah. Collaboration with BHS psychiatrists, psychologists, care coordinators, and facility staff is required. Documentation is completed remotely in the Athena I EMR. Requirements include a Master’s degree in Social Work, Counseling, or an eligible master's program and Utah licensure (LICSW/LMFT/LPC); NPI; Medicaid/Medicare credentialing a plus; vaccinations/CPR/First Aid; and Spanish language ability a plus. Required Qualifications
- Master of Social Work, Counseling, or other eligible master's degree
- Utah licensure as LICSW, LMFT, or LPC
- NPI (National Provider Identifier)
- Credentialed with Medicaid/Medicare a PLUS
- TB Test
- CPR/AED/First Aid Certification
- Spanish language ability a plus
- Basic computer knowledge
- Documentation in EMR (Athena I)
- Experience in geriatric population in SNF/LTC settings
