Job Summary
BRIDGES Navigator at Children’s Home Society of Florida leading intake and assessment activities, guiding families to health and human service resources, coordinating referrals and linkages, advocating for families, and supporting program documentation. Responsibilities include identifying client needs, conducting screenings, presenting information about available programs, maintaining client records, coordinating transportation and home visits, collaborating with community providers, training and mentoring staff/volunteers, and contributing to a positive team environment. Requires a Bachelor’s degree in Social Work or related field, a Florida driver’s license with reliable vehicle access, and two years of experience with children and families; role includes in-person work in West Palm Beach, FL and potential home visits. Strong communication, planning, organization, teamwork, confidentiality, and cultural competency are essential, as is commitment to CHS values and high-quality service delivery.
Required Qualifications
- Bachelor's degree in Social Work or a comparable Human Services field from an accredited university, required.
- Florida Driver’s License within 30 days from hire with daily access to a reliable and insured vehicle, required.
- Two years working with children and families, required.
