Visit patients in their homes to provide peace of mind and educate families about available healthcare services. Connect patients with community resources and assist in transitioning them between care services, including hospice. This role supports aging patients through the Bridge Program, requiring healthcare experience, a valid driver's license, and the ability to work independently. Schedule is Monday-Friday in Anchorage, AK, with a salary of $19-$22/hour.
