Job Summary
Brain Injury Case Coordinator responsibilities include coordinating supports for individuals seeking Brain Injury Waiver services, helping with referrals, intake, and eligibility, developing preliminary care plans using person-centered planning, coordinating and negotiating services with Brain Injury providers to implement care plans, assisting applicants with completing the Brain Injury Waiver Application, and documenting the status of Brain Injury Waiver applications; position requires Bachelor's in Social Work or related field with LSW eligibility, statewide travel, and partial telework with management approval.
Required Qualifications
- Bachelor's Degree in Social Work or related social service/social welfare area with at least 12 courses in behavioral science, social science, or social work
- Eligibility for conditional or full licensure as Licensed Social Worker (LSW) as determined by the Maine State Board of Social Worker Licensure
- Possession of a valid Class C Maine Motor Vehicle Operator's License
- 1 year of experience working with adults with Brain Injuries and at least 1 year of case management experience
- Submit transcripts with application materials
- Experience in developing preliminary care plans and coordinating services with providers
