Job Summary
The Home Health Nurse (RN/LPN) will manage and deliver comprehensive home health services, conducting assessments, interventions, and supportive care to clients at their residence. Key responsibilities include patient assessment, care planning, care coordination, and effective communication with patients, families, and healthcare teams. The position requires acute medical/surgical nursing experience and includes roles in patient/caregiver advocacy and telephone triage. Proper licensure and certifications are mandatory for this role.
Required Qualifications
- Massachusetts RN Licensure
- Valid state issued Driver’s License
- One (1) year of acute medical/surgical nursing experience
- Cardiopulmonary Resuscitation (CPR) Certification
