Job Summary
Conduct comprehensive case management for 15 residents and 4 families, including engagement, intake, assessment, individual service planning, and ongoing monitoring. Maintain documentation in accordance with regulatory policies, including weekly progress notes, data entry into HMIS, and completion of periodic reports for The Salvation Army, LAHSA, and HIMIS/Clarity. Collaborate with clients, mental health workers, and external agencies to develop housing plans, facilitate life skills training, and support discharge processes to transitional or permanent housing. Perform room inspections, mediate conflicts, and provide follow-up case management at 2, 4, and 6 months post-discharge. Maintain an on-call status for emergency responses and supervise house monitors while ensuring adherence to safety and security measures.
Required Qualifications
- Valid California driver's license
- Pass a criminal background check
- BA in human services field with minimum of two years of experience
- Four years of social services experience with case management
- Ability to keep organizational and client confidentiality
- Computer literate and proficient; able to use computerized database system for information management
- Supportive of The Salvation Army's Christian mission and philosophy
- CPR/First Aid Certification
- Culturally sensitive and non-judgmental regarding issues with homelessness, mental health illness, substance abuse, etc.
- Ability to carry out assignments independently and strong time management skills
- Knowledge of mainstream benefits, homeless assistance, employment, housing, medical and mental health resources in the community
- Professionalism in relation to clients, fellow employees, and community contacts
- Excellent human relations/communication skills; ability to act in a professional manner regardless of circumstances
- Skilled in non-violent crisis intervention
- Ability to self-motivate and organize time effectively
- Must be able to lift up to 25 lbs
