Job Summary
Independent Licensed Therapists (LCSW, LMFT, LPC) are sought to join a geriatric SNF/LTC therapy team in Idaho. Responsibilities include completing diagnostic evaluations, providing psychotherapy, developing treatment plans, and consulting for holistic patient care; in-person service at partner SNF/LTC facilities; collaborate with psychiatrists, psychologists, care coordinators, and facility staff; document promptly in the Athena I EMR; maintain compliance with agency policies. Qualifications include Master’s-level degree and Idaho licensure as an LCSW/LMFT/LPC with an NPI; Medicaid/Medicare credentialing is a plus; vaccination/TB testing, CPR/AED/First Aid certifications; Spanish language ability is a plus. Required Qualifications
- Master of Social Work, Counseling, or other eligible master's degree and Idaho licensure as an LCSW, LMFT, or LPC

- CPR/AED/First Aid Certification
Desired Qualifications
- Independently Licensed Therapists (LCSW, LMFT, LPC) with Idaho licensure
- Experience providing diagnostic evaluations, psychotherapy, treatment planning and consulting in SNF/LTC settings
- Proficiency with EMR systems (Athena I)
- Bilingual abilities in Spanish (preferred)
- Strong interpersonal and documentation skills
- Ability to work in geriatric population across Skilled Nursing and Long-Term Care facilities
- NPI (National Provider Identifier)
- Credentialing with Medicaid/Medicare (a PLUS)
- Vaccinations/TB testing compliance
- CPR/First Aid Certification
