Job Summary
Complete 36-40 hours of classroom instruction and pass all tests with an 80% score. Perform 75 hours of supervised clinical practicum in a long-term care facility, including the first 16 hours with the class instructor. Demonstrate required skills on RCP checklists and submit signed daily clinical sign-in records. Pass a criminal background check, TB test, and physical prior to starting. Maintain mandatory attendance to meet Indiana State Department of Health program requirements.

- Pass all tests administered during the classroom portion with 80%
- Have 3 attempts to pass classroom tests
- Complete 75 hours of supervised practicum in a long-term care facility
- Perform all required skills successfully during practicum
- Attend all classroom sessions
- Complete a clinical sign-in record daily with date, actual hours worked, student signature, and cosignature by a licensed supervisor
